Complete or gather the following forms and save to your computer or device. You will upload these items when you apply through CSS:
- Application - Grant Deed
- Application - Supplemental
- Owner Authorization Affidavit
- Business Registration - If applicant is a business. Can apply at end of submittal process.
- Calculations - Building Square Footage
- County Hazardous Materials Questionnaire (Commercial & Mixed Use only)
- Form - Construction and Demolition Debris-Waste Management Plan
- Form - Stormwater Intake Form and Standard SWQMP
- Owner-Builder Information and Acknowledgment
- Letter - SDGE Disconnect (if building has electricity)
