• Application Supplemental
• Authorization to Release Information
• Business Registration
• Plans - Site Plan - Regulatory
• Proof of Ownership or Permission from Owner
• Senior Citizen Association or Mobilehome Park Association By-Laws
• State Franchise Tax Board (FTB) Letter of Exemption
